ASSUMPTION OF RISK
I understand that my assigned activities may include work that may be hazardous to me, including but not limited to: cooking, food handling, driving, demolition, construction, loading and unloading activities, and transportation to and from the work sites. I hereby expressly and specifically assume the risk of injury or harm inherent in any of my assigned activities and release the HLCN from all liability for injury, illness, death, or property damage resulting from these activities.

RELEASE AND WAIVER
I hereby release and forever discharge Ark of Highland Lakes from any and all liability, claims and demand of whatever kind either in law or in equity, which arise or may hereafter arise from my activities with Ark of Highland Lakes. I understand that this Release discharges Ark of Highland Lakes from all liability or claim that I may have against Ark of Highland Lakes with respect to bodily injury, personal injury or property damages that may result from my activities with Ark of Highland Lakes. I also understand that Ark of Highland Lakes does not assume any responsibility for or obligations to provide financial or other assistance, including but not limited to medical, health, auto or disability insurance in the event of injury or loss.

MEDICAL
I understand and agree as a volunteer that this Release discharges Ark, from any liability or claim that I may have against Ark with respect to any bodily injury, personal injury, illness, death, or property damage that may result from my activities with Ark, whether caused by negligence of Ark or its representatives, volunteers, officers, directors, employees, or agents, or otherwise.

INSURANCE
I understand that Ark does not carry or provide health, medical, disability or auto insurance coverage for any emergent volunteer; therefore, Ark does not assume any responsibility for and has no obligation to provide financial or other assistance, including but not limited to medical, health or disability insurance, in the event of injury or illness.

CONFIDENTIALITY
I understand and agree that, in the course of my participation as a volunteer, I may have access to information or security related items (i.e. keys, combinations, etc.) that are confidential due to security concerns. I understand and acknowledge that I will not disclose this information or any other security-related information to any person without prior approval of my supervisor

PHOTOGRAPHIC RELEASE
I grant Ark, its successors, assigns, and licensees, the perpetual right to photograph, film, use and reproduce, as Ark desires, photographs and videotapes taken of me and/or my children during any volunteer activity. I understand that I will not receive any compensation for my participation or my children's participation in the photographs and videotapes and that the Ark shall own all right, title, and interest to the photographs and videotapes, including the portions that contain the images and voices of me and/or my children.

OTHER
I agree that this Release and Waiver is intended to be as broad and inclusive as permitted by local and state laws. I agree that in the event that any provision(s) of this release shall be held to be invalid by any court of competent jurisdiction, the other portion(s) will remain in effect.
